Medications
Prescription Medications
1. There must be a written request from the child's physician which includes: name of student, medication, dosage, time to be given and dates to be given in school. The request should be dated and include the physician's name.
2. There must be a written request by the parent or guardian that the medication is to be given as prescribed in the physician's request.
3. Medication must be in the original container labeled with the child's name and the name and dosage of medication.
Non-prescription medications
1. Students are permitted to take non-prescription medications at school when they are accompanied by a written request from the parent or guardian. The written request must include student's name, date(s) to be given, medication name, reason, dosage, time to be given, and the parent's signature.
2. Medication must be in the original container.

Standing Order Medications
Students whose parents/guardians have signed permission on the Health History Update Form may take Tylenol, Mylanta or Maalox, cough drops or throat lozenges. You may also sign permission for use of Oragel.
